Social Prescribing

Social Prescribing is a holistic approach to healthcare that brings together the social and medical models of health and wellness, providing a formal pathway for healthcare providers to address the diverse factors of health.

Bridging the gap between clinical and social care, a social prescription refers seniors to a Community Connector who will help them write an individualized wellness plan that connects them to local, non-clinical services chosen to support their interests and goals.

What does a social prescription look like? It could be connecting to a walking or exercise group, meeting others who like to play cards, learning how to access food supports, finding a volunteering opportunity and much more. Contact your health practitioner to get your Social Prescription and get a hand in Healthy Aging.

Healthcare providers can submit a referral to our Community Connector by completing our Social Prescribing referral form.

Food Hampers

Our Food Hampers program is designed to support seniors by providing essential food items, ensuring they have access to nutritious meals. These hampers, delivered by caring community volunteers, include a variety of fresh produce, canned goods, and other staple items.

The program not only helps to alleviate food insecurity but also offers a warm connection to the community, reminding seniors that they are valued and supported. Through this service, seniors can maintain their health and well-being, knowing that they are not alone.

Social: Penpals

Through the exchange of letters with penpals, seniors can experience meaningful connections and build lasting relationships. This provides emotional support, companionship, and a sense of belonging, helping to combat loneliness and  improve the overall well-being and quality of life for elderly individuals.

A heartfelt letter can go a long way in ensuring seniors feel connected and cared for.
